Reliance life insurance misselling and fraud complaint for policy no 19865669

I, neelesh ahirwar, policy holder of policy no. 669, cheated by mr. Avinash bhargav and mr. Vishal choudhary on behalf of reliance life insurance company ltd. Mr. Avinash bhargav called me on 10-02-2012 and introduced himself as insurance agent of reliance life insurance office, delhi. He offered me the following plan: I have to pay five installments of rs. 00/- Premium for five years. And the plan has following benefits: 1. On completion of 6th year the invested amount will be rs. 00/- 2. Plan has the option of medical insurance. In which I can register any five members of my family for medical treatment of rs. 00/- Per annum. 3. 30% of the 1st installment will be refunded in 6 months I.E. Rs. 0/-, If I take the policy on same day, it was 012. 4. I can withdraw rs. 00/- On completion of 6th year and the balance amount of rs. 00/- Will be providing continue medical for my family members. Or I can withdraw whole amount of rs. 0/-, If I don’t want the medical benefits after 6 years. 5. Policy will return 15% of sum assured from year 1-14 and 40% of sum assured in 15th year respectively. After confirming the above points from mr. Avinash, I agreed to take the policy and handed over cheque of rs. 00/- Dated 10-02-2012 to reliance’s collection executive and signed the application form no. C5453763. I wrote my family members name age/Date of birth behind my driving license/Pan card xerox copy, which I submitted with application form for my permanent address proof, if you need a proof you can verify it. After receiving my documents mr. Avinash verified my family members details on phone too. After that I waited for long time till april-2012 but no policy documents were delivered to me. I was following up the same with mr. Avinash, but there was no effect. In MID of april-2012, I called customer care asking for the policy documents, customer care executive told me to send an email from your registered email ID for the same. I sent the email and a copy policy documents were delivered to me on 012. Since there was no document stating that it is having any kind of medical benefits for my family members. Again to confirm the same I called up mr. Avinash, and explain him the same, he convinced me the medical benefits documents will be issued separately in 10 days. And another person, vishal choudhary, from the same office called me, that he received a complaint regarding medical benefit documents are not being issued to me, and he is forwarding my request to hyderabad and he will look into the matter arrange to delivered the medical related papers in another 20 days. He convinced me on phone on 01/05/2012 for 30 minutes. I said ok, I will wait for another 20 days. Now it is june-2012, for the whole month I am continuously asking for the medical health cards, they said it will come soon and slowly they started ignoring my phone calls. On 012, somehow my phone got connected with mr. Avinash, I asked him if there are no medical benefits please cancel my policy and refund my money to my account. For that on 05-06-2012 he gave the following numbers. Request no. : B18732125 complain no. : 1278 to confirm, if these numbers are real of fake, I called customer care, the executive told me that there is no complain and request for my policy with the above request and complain numbers. Below are the names and contact numbers of both of them. 1. Avinash bhargav, core manager – 454, 011, 070, 1039 (M/S probus insurance broker limited) 2. Vishal choudhary – 847, 1055 (M/S probus insurance broker limited) after these misstatement and false commitment from insurance agent on behalf of reliance life insurance company ltd., I am no more interested to continue the policy. During this four months period I have already spent more than rs. 00/- For my family members medical treatments. I need my money back to have some other health care plan from other company. Please assist me on this problem.
