Tibros study table received defective and yet to be collected by Star CJ

Tibros study cum bed table was delivered to me on 11-04-2012 in defective state via order number 4728 and I had filed a complaint for replacing/Returning the table on the same date. The product was due to be collected from me by 18-04-2012 but even after repeated calls and speaking with 3 senior executives, the product is yet to be collected from me. For filing complaint with consumer forum, a weeks notice is to be given to them but they are not providing me the address to mail my complaint. Kindly help me out in this matter.
